If you have a copay or coinsurance plan, you will likely only be responsible for $30-60 per office vist. Evaluations usually include 3-4 office visits, so your total cost might range from $90-$240.
We will always verify all insurance before your first appointment, and that will give us a better idea of what your insurance provider may cover. We will provide you with an email with our estimated breakdown for each appointment prior to your intake appointment.
The best way for us to get the most accurate estimate possible is if you make sure to provide pictures of your insurance card at the time of scheduling.
Fees are due at the time of service. You are responsible for paying your share of each appointment, based on our verification of benefits, on the day of the appointment.\n\n[/cs_content_seo][/cs_element_accordion][cs_element_gap _id=”28″ ][cs_element_headline _id=”29″ ][cs_content_seo]Insurance Estimate Disclosure:\n\n[/cs_content_seo][cs_element_gap _id=”30″ ][cs_element_text _id=”31″ ][cs_content_seo]We do our best to provide you with accurate information given to us by your insurance company. However, we cannot be held responsible for any estimates given as the final decision is made by your insurance carrier after the claim is received.
If you have any questions about our estimate, please contact your insurance provider directly to verify your coverage. Your insurance policy is an agreement between you and your insurance company, and since we are providing services to you and not your insurance, all charges are ultimately your responsibility. \n\n[/cs_content_seo][/cs_element_layout_column][/cs_element_layout_row][/cs_element_section][cs_element_section _id=”32″ ][cs_element_layout_row _id=”33″ ][cs_element_layout_column _id=”34″ ][x_prompt type=”left” title=”Interested in Scheduling?” message=”Click here to request a call from our client care team who can walk you through our services and the scheduling process!” button_text=”Request Your Call Now!” button_icon=”calendar-alt” circle=”false” href=”/book” href_title=”” target=”blank” class=”man”][/cs_element_layout_column][/cs_element_layout_row][/cs_element_section][/cs_content]
